PHTS is committed to protecting your privacy and committed to developing technology that gives you the most powerful and secure online experience.
We provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available on our homepage and at every point where personally identifiable information may be requested.
Billing and personal information is encrypted whenever transmitted or received online. Personal information is accessible only to staff, agents, or contractors of PHTS. This privacy statement applies to all PHTS-owned web sites and domains. This privacy statement covers personally identifiable information, anonymous data collection and aggregate reporting. What we collect
During the registration process, the types of personal information you provide to us may include, name, address, phone, fax, email address, social security number, license number, date of birth, username and password, billing information, transaction, and credit card information.
When you are logged in to MyPHTS, in addition to the information already listed above, the types of personal information you provide us may also include personal health information (PHI). MyPHTS contains information and communication on selected limited PHI and does not reflect your complete medical record which you can request from your individual healthcare providers. All information and communications within MyPHTS are subject to all applicable state and federal laws governing PHI security and confidentiality.
How we use it
We use your personal information for the following purposes:
- To make the site easier for you to use by not making you enter your personal information more than once.
- To deliver services that you request or purchase.
- To help you effectively complete your educational requirements.
- To help us create and publish content most relevant to you.
- To alert you of product upgrades, special offers, updated information and other new services provided from PHTS.

Who we share it with
We never sell or rent your personal information. PHTS may disclose your personal information if required to do so by law (for example, a subpoena) or regulation, or in good faith to (a) provide MyPHTS services as described in PHTS Terms and Conditions and permitted by law, or (b) comply with legal processes served on the site, or (c) protect the rights and property of PHTS, or (d) where our records indicate fraudulent activity or other deceptive practices that a governmental agency should be made aware of, or (e) where your communication suggests possible harm to others.
PHTS may transfer information about Users if PHTS is acquired by or merged with another company. PHTS is not responsible for notifying User of such changes.
When you register, PHTS will not share your information with third parties without your permission, other than for the limited exceptions already listed. It will only be used for the purposes stated above. We use personal information to reply to inquiries, handle complaints, provide operational notices, keep records up to date to notify you via periodic e-mails of technical service issues, specials, or other related product/service information. Your information may be shared with agents or contractors of PHTS for the purpose of performing service for PHTS.
The online registration at PHTS is designed to give you options concerning the privacy of your credit card information, name, address, e-mail and any other information you provide us. PHTS is committed to data security with respect to information collected on our site. We offer the industry standard security measures available through your browser called SSL encryption.

Your personal information is never shared outside the company without your permission, except under conditions explained above. Inside the company, data is stored in password-controlled servers with limited access.
Through the registration process, you create a unique username and password to use in logging in to MyPHTS.
Only authorized individuals may access personal health information (PHI). If you are not an authorized individual, access of PHI is prohibited. Your unique username and password permits us to identify and monitor user activity for improper access of PHI.
MyPHTS automatically logs you out terminating your access after a predetermined period of inactivity. Your unique username and password has to be entered in to the log in every time the application is reopened to view or respond to information and communication contents.
We will provide you with the means to ensure that your personal information is correct and current. You may review and update this information at any time by logging into your account.
You also have a significant role in protecting your information. No one can see or edit your personal information without knowing your username and password, so do not share these with others.
Do not include personal health information or questions about your personal health information in emails to the PHTS team. The contact email provided on the MyPHTS site is for contacting PHTS support for MyPHTS membership and website access related issues. Contact your provider directly for your personal healthcare or personal health information concerns.
To protect your privacy and security, we will also take reasonable steps to verify your identity, such as password, username, email address, and or registration codes, before granting access to your data.
Integrity and Encryption
We protect the integrity of sensitive data and personal information from unauthorized alteration or destruction using the SSL industry standard security measures. We use SSL encryption when collecting or transferring sensitive data such as credit card and personal information. Credit card numbers are only used for processing payment and are not used for other purposes. Our efforts to protect credit card fraud help protect your financial data security.
Sensitive data and personal information stored in MyPHTS is encrypted to also prevent unauthorized access and disclosure. No non-encrypted sensitive data or personal information is stored in MyPHTS.
Can I Update my Personal Information?
You should be aware that it is not technologically possible to remove each and every record of the information you have provided to us from our systems. The need to back-up our systems to protect information from inadvertent loss means that a copy of your personal information may exist in a non-erasable form that will be difficult or impossible for us to locate. We promise that promptly after receiving your request, all personal information stored in databases we actively use and other readily searchable media will be updated, corrected, changed or deleted, as appropriate, as soon as reasonably practicable.
Ownership and Controls
MyPHTS contains information and communication on selected limited PHI and does not reflect your complete medical record. MyPHTS information are not designated record sets as defined by applicable state and U. S. federal laws. Where and when needed by the member, the complete medical record PHI should be requested from the individual member’s healthcare providers.
All pages within the MyPHTS site and any information made available through MyPHTS are the property of PHTS. The content on the MyPHTS site is provided for registered users only. Information and communication content in MyPHTS will be available for viewing on a selected, limited basis determined by PHTS and as governed by applicable state and U. S. federal laws.
Accepting an invitation to register for access to use MyPHTS is voluntary. Care Coordination Members may cancel their access to use MyPHTS account by requesting access cancellation. Upon cancellation, such members will no longer be able to access or use MyPHTS.
Sponsor registration and Care Partner registration are voluntary. Accepting an invitation to register for access to MyPHTS is voluntary. Some information may be stored with an expiration date. You will be notified of the expiration date of such information by displaying the expiration date on MyPHTS. PHTS may permanently delete personal information stored with an expiration date as of the expiration date.
PHTS has no obligation to return personal information to you after your account is cancelled or personal information stored with an expiration date has expired and has been permanently deleted.
De-identified information, which will only be in general terms and will not contain any personal or identifying information about members, may be retained, used, or disclosed in order to create comparative databases or to perform statistical analysis and for such other lawful purposes.
Our web sites are not setup to track, collect or distribute personal information. Our site does generate certain kinds of non-identifying site usage data, such as number of hits and visits to our sites. This information is used for internal purposes only. The statistics contain no personal information and cannot be used to gather such information.
